Mix and shape the patties
In a large bowl, stir together the egg, bread crumbs, 2 tablespoons of the onion soup mix, and a good dash of pepper. Crumble the ground beef in and mix lightly but thoroughly — don't overwork it or your patties will turn out tough. Shape into 4 oval patties.

Brown the patties
Heat a large cast-iron or heavy skillet over medium heat. Cook the patties 4-5 minutes per side, until well browned and cooked through (160°F in the middle). Move them to a plate and keep warm.

Soften the onion
Slice the yellow onion thin. Add the butter to the same skillet and cook the onion over medium heat for 5-6 minutes, stirring now and then, until soft and golden. Scrape up any browned bits from the bottom of the pan as you go — that's where the flavor lives.

Build the gravy
Whisk together the flour, water, Worcestershire sauce, and the remaining onion soup mix until smooth, then pour it into the skillet with the onions. Bring to a boil, then reduce heat and simmer, stirring, until thickened, about 5 minutes.

Bring it together
Slide the patties back into the skillet and spoon the onion gravy over the top. Let everything warm through together for a couple of minutes, then serve hot over mashed potatoes.
